Case Management & Social Connectedness

At Daily Bread, Inc., we ensure that no one in Brevard County faces hunger or homelessness alone. We are a partner with our hungry and homeless neighbors as they make their journey to self-sufficiency. Important aspects of being a partner are case management and social connectedness.

Case Management
Our team of professionals work with our clients on a 1:1 basis to help them navigate the hard and windy road to self-sufficiency. After completing an assessment, we are able to determine which social supports are needed and we work with our community provider partners to connect our clients to the needed services. We work with our clients to develop an individualized plan to help them overcome barriers and make marked progress their identified goals.

Social Connectedness
Many of our clients who are facing hunger or homelessness also experience a tremendous amount of shame and isolation. There may also be, at times, competing personal or mental health issues that lend to one’s feeling of isolation. Our social connectedness programs are aimed at alleviating those feelings and helping our clients re-engage in the community, into their families, and back to work. These programs include Alcoholics Anonymous (AA); Narcotics Anonymous (NA); Prayer Group; Men’s Fellowship; and Women’s Fellowship.
